EQUAL ACCOMMODATIONS DISCRIMINATION COMPLAINT

If you believe you have been denied, refused or withheld the services, facilities, privileges or advantages of a place of public accommodations because of your race, age, marital status, creed, color, sex, national origin or disability, you can file an Intake Equal Accommodations Discrimination Complaint form. You can also contact our office by telephone. You have 90 days from the date of the alleged incident to file a complaint with our office.

HOUSING DISCRIMINATION COMPLAINT

Housing discrimination is a violation of a basic right, which assures all persons, regardless of race, age, marital status, creed, color, sex, religion, national origin, familial status, or handicap equal opportunity and equal access to live wherever they choose, if qualified. If you believe you have been discriminated against, you can file an Intake Housing Discrimination Complaint form. You can also contact our office by telephone. You have one year from the date you believe you were discriminated against to file a complaint.

INSTRUCTIONS FOR FILING A DISCRIMINATION COMPLAINT

To file a discrimination complaint, you can file the complaint online or print out (download) the form, fill it in manually, and then mail it to the Office of Human Relations.

If you do not have an e-mail client (i.e. e-mail capabilities) and you want to file your complaint online, you should use one of the following forms:

  • Intake Housing Discrimination Complaint Form  Temporarily Unavailable

Just fill in the form, and then click the "Submit Complaint" at the bottom of the form. If you want a printed copy of your complaint, you should use your browser's print feature to do a screen print before you submit the complaint. Note that an e-mail address is requested on this form. If you do not have one, just enter "None" in this field.

This type of document can be viewed, filled-in, printed, and saved to your computer for future reference. Fill in the PDF form, print and/or save the filled in form, if you so choose, and then click "E-mail this Form" at the bottom of the form. A copy of your completed complaint will be attached to an e-mail addressed to the Office of Human Relations. If you have additional pages to send with your complaint, you can also attach them to this e-mail or to the pdf file before sending it.
